COMPLETE ORTHODONTIC RECORDS PACKAGE

This complete package includes everything you will need to present the case to your patient. Whether treating an InvisAlign®, minor tooth movement, or a full Orthodontic case, proper documentation is not just important, it’s essential.
The Appliance Review Letter can also be requested with the records package. Our will diagnose and provide possible options to help you treatment plan your case. The Appliance Review Letter includes a written summary of the patient’s skeletal and dental conditions, illustrated appliance design(s), a preprinted prescription slip and a $10.00 appliance voucher.
