Q: How can I send digital patient records / images?

A: You can either send digital patient images by utilizing our Submit a Case Online feature, or you can send them on a CD if you intend on sending the case in the mail. To Submit a Case Online, you will need to log in to our website. If you have yet to create an online account, registration is quick and simple. Once you are successfully logged on, click on Submit a Case Online within the Second Opinion section of our website and follow the steps

Q: How recent should the records be when sending in a case for evaluation?

A: Typically records should be no more than 3 months old.

Q: Can I receive my report in a Digital format (PDF file)?

A: Yes. If utilizing the Submit a Case Online feature, you will have the option of selecting Digital and/or Printed Report. If sending a case through the mail, select Digital Report on the Patient Profile form. Click on Order Forms for further information.

Q: How do I access my patient’s Digital Report?

A: Click on Digital Report Archive which is located in the left column under Online Tools (Note: Requires login).

Q: What is the Appliance Review Letter and Orthodontic Diagnostic Service?

A: Sometimes you may need assistance in selecting the best appliance to treat a Minor Tooth Guidance case. For these types of scenarios, the Appliance Review Letter can be requested. When a more in-depth analysis is required or when you intend on sending complete records of your patient (models, panorex x-ray, cephalometric x-ray, photographs, etc.), we would recommend the Orthodontic Diagnostic Service. This comprehensive report includes step-by-step instructions on how to treat a particular case.

Q: What is the difference between the Appliance Review Letter and Orthodontic Diagnostic service?

A: First, it depends upon the type of case it is. Second, it depends upon the type of records you intend to send for evaluation. The Appliance Review Letter is to assist you with single-phase, minor tooth guidance cases when utilizing upper and lower models for evaluation.  If additional records are being sent for evaluation (i.e. cephalometric film) or if a case needs multiple phases requiring orthodontic and/or orthopedic therapies, refer to the Orthodontic Diagnostic Service.

Q: How can I check the status of my patient’s case?

A: For all Second Opinion related cases, click on “Case Archive” on the left column or call 800-423-3270 and ask for the Second Opinion Department. NOTE: Currently, the “Digital Report Archive” option is available only if the “Submit a Case Online” feature has been utilized or if a Digital Report was selected on the Patient Profile form

Q: Should I send in wet impressions or stone models?

A: We do not recommend sending wet impressions due to leakage concerns and likelihood of distortion of the impression material. When sending in models, send white stone models (preferred) and wrap the models and wax bite separately and carefully so as to avoid breakage.

Q: Who evaluates the case when a Phone Consultation is requested?

A: The appliance Design Staff at Space Maintainers Laboratory.
