GETTING STARTED
Step by step ordering instructions.

To expedite your order, we have outlined the steps that are required to complete your request. For those of you that utilize digital imaging in your practice, many of our services can now be fulfilled directly from your computer. However, depending upon the type of service that is requested, upper and lower working models may need to be submitted in the mail.

To begin, determine the method in which you intend to send your order:

Forms

Fill-out a Patient Profile form to select the desired service . If the Appliance Design  or Orthodontic Diagnostic Service is requested, a Doctor’s Profile form must also be submitted. To obtain these forms:
Option #1 – Click here to Print the forms.
Option #2 – Call 800.423.3270 to have the forms faxed to your office.

Records

Prepare the necessary patient records for the type of service that has been selected – Refer to the Patient Profile form for further information.

Send

Ship the form(s) and patient records via a “traceable” carrier (UPS, FedEx, DHL, etc). This will allow you to track, locate and verify the arrival of your package to our office. Please use our UPS account #917125 and send to the following address:

Important Shipping Note:
If models and a wax bite are to be sent, send white models (preferred) and wrap the models and wax bite separately and carefully so as to avoid breakage. We do not recommend sending wet impressions due to leakage concerns and likelihood of distortion of the impression material. Ship patient records via a “traceable” carrier (UPS, FedEx, DHL, etc). We do not recommend that you send your cases via USPS. Send all records in one package!

How the Appliance Design and Orthodontic Diagnostic Service requests are processed:

Step #1 – Based on the records, Doctor’s Profile, Patient Profile and the type of evaluation that has been requested for that specific patient, Dr. Rob Veis will complete a comprehensive diagnosis. If we need further information to complete the evaluation or if there is a concern with the case, the Second Opinion staff will contact your office.

Step #2 – The diagnosis, in the form of a Printed or Digital Report, and all of your records (models, wax-bite, cephalometric x-ray, photos, panorex, etc.) are then returned to the doctor’s office. Appliances are not made at this time.

Step #3 – Doctor reviews the completed report. The treatment plan is then presented to the patient.

Step #4 – If the doctor decides to go ahead with treatment and appliance fabrication, new patient models are sent to Space Maintainers Laboratory.
